SUPPORTIVE SERVICES

(Case Management & Wellness)

Building the Foundation for Your Future

Our staff provides one-on-one support to help youth achieve their self-identified goals and navigate personal challenges:

  • Individualized case management and goal planning with a trauma-informed approach
  • Assistance obtaining vital documents (birth certificates, Social Security cards, state IDs) and accessing public benefits (food assistance, medical insurance, etc.)
  • Employment readiness coaching: resume building, mock interviews, and referrals to job training programs (through partners like Turnaround Tuesday)
  • Education support: help with re-enrolling in high school or GED programs, college/trade school applications, and tutoring/mentorship to stay on track
  • On-site and referral-based health services: periodic visits from healthcare providers (e.g., Health Care for the Homeless) for basic medical care, HIV testing, or mental health counseling, as well as linkage to off-site clinics
  • Life skills and support groups: We offer groups and workshops on topics such as financial literacy, healthy relationships (formerly run in collaboration with TurnAround, Inc.), parenting support for young parents, and trauma recovery (e.g., the S.E.L.F. group). (These activities help youth build coping skills and peer support networks.)

We believe your goals matter. YES will be your ally, advocate, and support system every step of the way as you navigate your journey to independence.
